Select the card that you would like to add a Lead capture to:                                                                                                                                                                                                                            

Select "campaigns":                                                                                                                                                                                                                                                                                                

Select the campaign that you would like a Lead Capture form on:                                                                                                                                                                                                                    \

Then select "Edit"                                                                                                                                                                                                                                                                                                

Select "Include Lead Capture":                                                                                                                                                                                                                                                                          

Once you have selected "Include Lead Capture" you can add what information you would like to collect, how often you would like to be notified of the lead capture being filled out and the placement of your Lead capture form.
